Gotta rush so I'll post quick:
Completion time - Approx. 5 hours
I'm still not done though.
C&C would be GREATLY appreciated.
I feel this is my best photomanipulation to date, however, I still need to work on it.
Please ignore the crap quality. I had to compress it to upload it as an attachment.

Wow! VERY well done. The flame in the eye is a nice touch. I've looked over your piece several times, and am taken by the textures as well as the colors. A nice piece, I like it a lot!

I'm glad everyone likes it.
The healing brush and me are officially enemies though. Why does it have to be so stupid when I'm nearly finished. ><
I'll post my progress as I go.
EDIT:
Added stitching above the right eye.

The stitches add a whole different quality/feel to the piece than just the crack. Quality work!

Hmm, how about this:
I want to use this as a wallpaper, just some small stuff that bug me:
- The flame in the eye looks out of place.
- The face if possible would be positioned a little more center (I know part of the face is missing, so to balance, perhaps move it a little left.)
- Not much for seeing text on it, but it is your work.. so I cannot really complain there.. lol
Phew, I believe this is my real first c&c.. not bad or a code whore.. o.0

I really like it! The different textures just set the whole image off but the off centre composistion does bug me a little. Just like order said try moving it over to the left a bit.

Wow! Thats really good!
But, IMHO, I would completely remove the fire from the eye and add a pupil instead.

I like the eyes too - they bring a bit of life to a piece otherwise filled with grungey decay. You need that element really, or else you'll end up depressing everyone!

Dont know if you noticed or not but you forgot to move your stiches over with the entire picture. Amazing work though. I have always been interested in work such as this and still am lost as one goes about creating such pieces. Any tuts i should be looking to go this route. Or was it a basic trial and error for you to get this far. please inform

I feel so stupid right now ... I can't believe I forgot to move the stitches lol.
@ Stiker - I might put up some tutorials concerning photo manipulation when I start up my new site. The best route to go would be to experiment. Learn how different layer styles change the image and learn how to use the dodge/burn/healing/cloning tool. Those are VERY helpful. They'll be your best friend (and worst enemy at the same time >< ).
Before this, I did some minor grunge work. One of the most things to consider is the amount of time you put into whatever you do. I took about 4.5 - 5 hours to do this piece. This is probably my most complicated piece to date.
As for tutorials, I'm not sure. I haven't seen any helpful photo manipulation tutorials. The majority of them cover things like recoloring a pupil. Simple things that don't help too much. My answer would be to experiment. Also, DO NOT use brushes (if you do keep it limited). Keep it limited. In my piece I didn't use a single brush outside from those included in Photoshop.
Would anyone be interested in me publishing a walk through?
