There is a Noise Reduction filter in Photoshop CS2 and above that may give you what you're after, but to be honest some degree of graininess can be quite attractive in a B&W image - so much so that some B&W conversion filters have a setting to add grain to an image! I think the main problem with this image is that the contrast needs a bit of tweaking - the sunlit road in the background is so bright that it draws the viewer's eye away from the intended focal point of the image (the child and flower), and the highlight on the child's hand is totally blown out.

PS: I'll move this thread to the Photography forum - I think it'll be more at home there.

Hope you don't mind, but I had a go at it myself. I cropped the image to remove most of the distracting background and then used Shadow/Highlight to even out the contrast. I used Noise Ninja to remove the worst of the graininess, and then finished off with a little selective dodging and burning.

There's nothing on that site related to photo manipulation - stop spamming

To be honest, I like the grain in the photo. It gives the photo a nice retro look.
