yeah, instead of using JUST the smoke, try using different images along with it, like water...try the plastic wrap filter, stuff like that
basically you'll have to create a displacement map mixing in some water texture with an overlay if you want to get it perfect, otherwise try 3d :P

i saw a commercial on tv and went to the website to see if i could get some pics. these are pretty close to what i was talking about,of course there would be more detail on the face and whatnot.
Krill...if the images you posted are from a tv commercial then i'd have to say that they are actually 3d... as far as i can tell...
to do something of this nature in photoshop would take a lot of time to setup some displacement maps to help distort the water texture properly...
or to hand paint as mentioned through the link tamlin supplied.
it can be done, it just needs time and patience.
if it was me, I'd create a 3d character and render out the water that way...

i've seen it done in photoshop a long time ago but i cant remember the exact steps. it DOES involve making a displacement map and it really wasnt too many steps. there were a few steps to be sure,but not real long.
i was hoping that maybe someone has run into the technique or would know and show how to do it.it's a reallly cool effect that i'd really like to learn. thanks for helping though!
