There is a Noise Reduction filter in Photoshop CS2 and above that may give you what you're after, but to be honest some degree of graininess can be quite attractive in a B&W image - so much so that some B&W conversion filters have a setting to add grain to an image! I think the main problem with this image is that the contrast needs a bit of tweaking - the sunlit road in the background is so bright that it draws the viewer's eye away from the intended focal point of the image (the child and flower), and the highlight on the child's hand is totally blown out.
